Anticoagulant service in the community

Since 1 October 2019, Croydon GP Collaborative has delivered anticoagulation services in partnership with Croydon University Hospital, succeeding the previous provider, Boots. Our collaborative approach ensures patients across Croydon receive coordinated care across both primary and secondary settings.

From February 2026, the service has expanded to include home visits, improving accessibility and supporting patients in their own homes as part of a fully integrated care pathway.

 

Service aims

The aim of the service is to successfully deliver a truly integrated Anticoagulation service our vision is:

  • Right Place, Right Care: strengthening, enhancing and designing partnerships between primary and secondary care that drive quality and provide the most patient centred model of care
  • Right People: maximising opportunities by sharing knowledge and expertise between primary care and secondary care providers which is an enabler to developing system capacity and delivering high quality, faster care to patients

The service is runs from 9 GP practices, which were chosen due to their locations being as close to the branches of Boots who previously ran the service ensuring they were DDA compliant and had adequate public transport links. Ensuring disruption to patients is minimal with the change of providers of the service.
